- Just a reminder about Medication Policies
- Children are not to carry medication to and from school.
- Parents should complete and sign a medication form. The medication should be in the original container with appropriate label.
- If your child has food allergies, please have your doctor complete the “special dietary needs” form so our cafeteria staff and clinic are aware of your child’s needs.
- For students with food allergies that require an Epi-Pen, we have a special form for that. We would also like to have a photo of your child with this form.
- YCSD Annual Contract for Student administered Epi-Pen form.
- If your child has Asthma that requires medication, please have your doctor fill out the Asthma Action Plan form
- If your child is going to be absent or tardy, please do not forget to call the clinic or front office. We are concerned about your children’s health and safety. You will need to send a note with your child when they return to school.
- If your child has been ill, please don’t send them to school unless they have gone 24 hours without a fever, vomiting, diarrhea, or treatment with an antibiotic.
- Let us know if your child has any health changes since last year. We also need and changes in your home, work, or cell numbers and emergency contacts.
- The clinic is always in need of sweatpants and shorts or pants with elastic that your child has outgrown. If you would like to donate clothing, send them to the clinic. We thank you!
